Adoption Agreement

SPAY/ NEUTER:

If the dog is not spayed/ neutered the Adopter agrees to have this surgery performed at six months of age unless discussed with a veterinarian and approved by Pup Culture Rescue. I understand that this spay/ neuter procedure will NOT be covered by Pup Culture Rescue and is the financial responsibility of the adopter. I agree to email records as proof of spay/ neuter once completed. 

ADOPTION FEE:

6 months & Under: $550 + $50 spay/neuter deposit ($600 upfront)

1 & Under: $550 (+ $50 spay/neuter deposit, if applicable)

2-6 years: $450

7+: $300

Adopter hereby agrees to pay the non-refundable adoption fee by 7th day of obtaining the dog. If adopter fails to pay the fee on time, they will incur a $20 late fee for each day that they are overdue. The rescue has the authority to reclaim the dog if the adoption fee is late.

I understand that this is a donation that will go towards the care of future rescue dogs and does not go directly to this dog’s expenses.

*For puppies six months and under who are not spayed/neutered, Adopter agrees to pay a $50 refundable deposit in addition to adoption fee.

UPDATE:

Adopter agrees to email or text message adopting agent/coordinator within two weeks of signing contract with an honest update of the newly adopted dog’s progress. 

TRAINING:

We strongly recommend that all adopters schedule a consultation with a professional trainer to ensure dog properly adjust to their new home. Not all dogs show behavioral problems initially. The dog’s temperament may or may not have been evident during its stay with us. Should any behavioral issues arise (I.e. biting, destruction, escapism, fearfulness, separation anxiety, etc.) adopter will immediately notify the adoption agent. Unless otherwise agreed upon in writing, Adopter agrees to provide all treatment and training at their own expense

CHIP & REGISTRATION:

Adopter agrees to license the dog immediately according to residing city laws. Adopters agree for the dog to wear a collar and ID tag with name and a minimum of one contact number at all times. In the case that the animal is not already microchipped, Adopter shall have the animal microchipped and agrees to keep the microchip updated with current address and phone number with Pup Culture Rescue as the secondary contact. This is listed as ID on the medical records.

*After submitting the adoption fee, the microchip information can be transferred by creating an account at my24pet.com and entering the dog’s microchip number (listed on their medical records). 

LOST/ STOLEN:

If the Dog is lost or stolen, the Adopter agrees to immediately contact the adoption agency. Adopter must also make an immediate and thorough effort to find dog, which includes but is not limited to: Filing a lost dog report with the local police, animal control authorities, and local veterinarians; Posting signs; Placing an ad on PawBoost, NextDoor, and CraigsList; Notifying the microchip company. If the dog is picked up by local law enforcement or animal control, the Adopter will make immediate arrangements to reclaim the dog.

GENERAL CARE:

Adopter agrees to care for the dog in a humane and responsible manner which includes, but is not limited to: Providing a safe home and proper care and attention;  A healthy diet and constant access to fresh water; Adequate exercise; Shelter at all times; Appropriate veterinary care. Adopter agrees that the dog is being adopted and treated as a member of their family and will NOT be used or trained as a guard dog or for any purpose other than a companion animal. Adopter further agrees that this animal will reside inside the home and shall NOT be kenneled or tethered outdoors for extended periods of time. Adopter agrees to adequately supervise this dog outdoors. In such times that the dog is outside, it shall be either on leash or in an adequately enclosed and secured area. Whenever transporting the dog, the Adopter agrees to do so in a manner that ensures the safety of the dog.

Adopter agrees to not perform any permanent, non-medical alteration to the dog's appearance - this is prohibited (I.e. docking of tails, cropping of ears, tattoos, removing dew claws, declawing, etc.) 

Pup Culture Rescue makes no representations or warranties nor any guarantee of this dog’s health, behavior, or breed. Any damage physically, or financially, that may incur will be the sole responsibility and liability of the Adopter upon execution of this agreement. 

RE-HOMING:

If, despite all best efforts, Adopter is unable to keep or provide adequate care for this dog, Adopter agrees to notify Pup Culture Rescue and discuss the best course of action to re-home the dog. Adopter agrees to foster the dog for up to three (3) months while working with Pup Culture rescue to find the dog a new home. Adopter agrees not to sell or give the dog to any other person, friend, relative, organization, laboratory, or animal shelter. Adopter is responsible for transporting the dog back to the adoption agency them-self regardless of their current location. Adopter agrees that adopting agent shall have the right of immediate possession of the dog if, in the judgment of the Adopting Agent, the animal is receiving inadequate care, is being improperly housed or handled, has not been spayed or neutered by the specified date, or if it is determined by adopting agent that repossession by Pup Culture Rescue is in the best interest of the dog. 

VACCINES:

Adopter has discussed the dog’s vaccine schedule with the adopting agent and has a full understanding. Adopter agrees that when a dog is not yet fully vaccinated/has pending vaccinations the dog must remain off of public grounds (I.e. sidewalks, hikes, beaches, the floors of both veterinary offices and pet supply stores) until all vaccinations have been completed and the dog is deemed up-to-date on all vaccinations by a veterinarian. Adopter agrees to contact the adopting agent with any questions or concerns about the dog’s vetting history.

MEDICAL:

If the dog develops a health problem within the first month with the adopter, the Adopter agrees to contact Pup Culture Rescue immediately before bringing the dog to a veterinarian. Pup Culture Rescue makes no promises, guarantees, or commitments to assist with medical care any time after adoption, but rather the decision will be made on a case-by-case basis. Pup Culture Rescue cannot consider assisting Adopter if Adopter contacts us after Adopter has taken the dog to a veterinarian. 

Due to the varying circumstances by which dogs are obtained, we have little or no medical history other than what is given to us. Due to the incubation period of certain diseases, viruses, and bacterial infections, it is strongly advised that Adopter keep newly adopted dog away from other animals for at least fourteen (14) days after adoption.

Some animal diseases can infect humans (zoonotic diseases). Examples of these diseases include rabies, roundworms, hookworms, fungal infections (I.e. ringworm, scabies, plague, toxoplasmosis, etc.) A veterinarian is an excellent source of information on the prevention of these diseases. The websites of the Los Angeles County Veterinary Public Health, the California Department of Food and Agriculture, and the Center for Disease Control are other sources of information. 

Adopter agrees not to euthanize the dog - except in the case of the a terminal illness or injury, or old age accompanied by pain and suffering, and in that case, the euthanasia must be performed by a licensed veterinarian. Adopter hereby acknowledges and agrees that Euthanasia should not be considered due to financial distress. 

WAIVER: 

Adopter is adopting the dog as is and expressed above. Adopter hereby releases Pup Culture Rescue from any and all liability for personal injury, property damage, legal fees, or veterinary care, or any other expense or liability incurred by Adopter as a result of this adoption or Adopter's possession of this dog. Adopter agrees to indemnify Pup Culture Rescue, its adoption agents, board of directors and volunteers, from any and all such claims and to pay, without limitation, any costs related to such injury, damage, or liability, including, in the case of litigation, any attorney' fees incurred by Pup Culture Rescue in its defense. Adopter also agrees and understands that the adoption is completed only upon receipt of the above adoption fee, completion of this agreement, and approval of this agreement by a Pup Culture Rescue Representative. 

With this adoption agreement, I understand that adopting is a permanent decision and a significant responsibility. I agree to take loving and responsible care of this animal for the duration of its life. I acknowledge the legality of this contract and agree to all the above terms and conditions.

All adopters have the option to exercise a 7-day trial period before submitting the adoption fee for their pup - once it is paid, it cannot be refunded. If you are confident this is the dog for you, and would like to pay right away, please go ahead!

A reminder that if you fail to honor the adoption fee on the 7th day of your trial, a $20 late fee per day must be included to your donation.
